ONLINE ADMISSION REQUIREMENTS AND PROCEDURES

GRADE 7 STUDENT

 

I BASIC ELIGIBILITY REQUIREMENTS

To qualify for admission to JHS, the student applicant must have;

  1. Completed elementary program recognized by the Government (Grade 6 completer).
  2. Submitted all admission requirements set by the college.

II APPLICATION PROCEDURES FOR JUNIOR HIGH SCHOOL GRADE 7

  1. The grade 7 student applicant may refer to the link below and fill out the basic information form.

  1. Then, submit the clear scanned image or picture copies of the following requirements to admissionoffice@mqc.edu.ph or mqcadmissionsandtestingoffice@gmail.com
  • Recent 2×2 picture with name tag (white background)
  • PSA/NSO Birth Certificate (Original)
  • Grade 6 Form- 138/ Report Card (Original)
  • Certificate of Good Moral Character

Send the requirements by the following format:

Subject: JHS Admissions Requirements

Message: Full name <space> Grade 7

Upon receipt of the scanned image/picture copies of requirements, the Admissions and Testing office personnel shall review all submitted documents.

  1. A schedule of interview will be given to grade 7 student applicant if the requirements are complete (The ATO, POD or SAO will notify the applicant of his/ her ONLINE/ PHONE INTERVIEW SCHEDULE through phone call/ they may also contact the parent or guardian of the applicant).
  2. The grade 7 student applicant will be interviewed by the JHS Prefect of Discipline or JHS SAO.
  3. After the interview, the Guidance and Counseling Office personnel will recheck the grade 7 student applicant’s requirements. After rechecking, the GCO personnel in-charge will issue permit to enroll and payment instruction.
  4. The parent/guardian of the grade 7 student applicant will present the permit to enroll and pay to any bank listed below.

  1. Once the payment has made, the grade 7 student applicant or parent/guardian will send the deposit slip to financeoffice@mqc.edu.ph (Upon receipt, Finance Office personnel will message the student applicant regarding the payment, including the procedures on how to access the student portal).
  2. Once the grade 7 student applicant able to access the MQC student portal, he/she is now considered officially enrolled.

GRADE 8 TRANSFEREE

  1. Completed grade 7 elementary program recognized by the Government.
  2. Submitted all admission requirements set by the college.

II APPLICATION PROCEDURES FOR THE GRADE 8 TRANSFEREE

  1. The grade 8 transferee applicant may refer to the link below and fill out the basic information form.

  1. Then, submit the clear scanned image or picture copies of the following requirements to admissionoffice@mqc.edu.ph or mqcadmissionsandtestingoffice@gmail.com

  1. After completing the requirements, the Admissions and Testing Office personnel will forward the requirements to the PRINCIPAL for evaluation of subjects. Then, the PRINCIPAL will send the list of subjects to ATO personnel.
  2. After the evaluation of subjects, a schedule of interview will be given to the grade 8 transferee applicant if the requirements are complete (The ATO, POD or SAO will notify the grade 8 transferee applicant of his/her ONLINE/ PHONE INTERVIEW SCHEDULE through phone call, they may also contact the parent or guardian of the applicant).
  3. The grade 8 transferee applicant will be interviewed by the JHS Prefect of Disciple or JHS SAO.
