ONLINE ADMISSION REQUIREMENTS AND PROCEDURES

NEW COLLEGE STUDENTS

 

I BASIC ELIGIBILITY REQUIREMENTS

To qualify for admission to College, the student applicant must have;

  1. Completed the basic education program from a school recognized by the Government (Grade 12 graduate).
  2. Submitted all admission requirements set by the college.

II APPLICATION PROCEDURES FOR NEW COLLEGE STUDENTS

  1. The student applicant may refer to the link below and fill out the basic information form.

  1. Then, submit the clear scanned image or picture copies of the following requirements to admissionoffice@mqc.edu.phor mqcadmissionsandtestingoffice@gmail.com
  • Recent 2×2 picture with name tag (white background)
  • PSA/NSO Birth Certificate (Original)
  • Grade 12 Form- 138 / Report Card (Original)
  • Certificate of Good Moral Character
  • Certificate of Academic Award (for applicants who graduated with honors)

Send the requirements following the format shown below:

Subject: College Admissions Requirements

Message: Full name <space> Year and Course

Upon receipt of the scanned image/picture copies of requirements, the Admissions and Testing office personnel shall review all submitted documents.

  1. A schedule of the interview will be given to the student applicant if requirements are complete (The ATO, POD, or SAO will notify the student applicants of their ONLINE/ PHONE INTERVIEW SCHEDULE through a phone call).
  2. The student applicant will be interviewed by the College Prefect of Discipline or College Student Affairs Office Head.
  3. After the interview, the Guidance and Counseling Office personnel will recheck the applicant’s requirements. After rechecking, the GCO personnel-in-charge will issue a permit to enroll and payment instructions.
  4.  The student applicant shall present the permit to enroll and pay to any bank listed below.

  1. Once the payment has been made, the student applicant will send the deposit slip to financeoffice@mqc.edu.ph(Upon receipt, Finance Office personnel will send a message to the student applicant and confirm the payment, including the procedures on how to access the student portal).
  2. Once the student applicant can access the MQC student portal, he/she is now considered officially enrolled.

 

COLLEGE TRANSFEREES & DEGREE HOLDERS

I BASIC ELIGIBILITY REQUIREMENTS

To qualify for admission to College, the student applicant must have;

  1. Previously been enrolled in any tertiary education institution recognized by the Government.
  2. Submitted all admission requirements set by the college.

II   APPLICATION PROCEDURES FOR COLLEGE TRANSFEREES & SECOND-DEGREE HOLDERS

  1. The student applicant may refer to the link below and fill out the basic information form.

  1. Then, submit a clear scanned image or picture copies of the following requirements to admissionoffice@mqc.edu.phor mqcadmissionsandtestingoffice@gmail.com

  • PSA/NSO Birth Certificate (Original)
  • PSA/NSO Marriage Certificate (Original) if married
  • Transcript of Records (Original) or Original copy of Grades
  • Certificate of Good Moral Character
  • Honorable Dismissal

Send the requirements following the format shown below:

Upon receipt of the scanned image/picture copies of requirements, the Admissions and Testing office personnel shall review all submitted documents.

  1. After completing the requirements, the Admissions and Testing Office personnel will forward the requirements to the INSTITUTE DEAN for evaluation of subjects to be taken. Then, the INSTITUTE DEAN will send the Crediting form to the ATO personnel.
